NOTE: All forms require a "wet-ink" signature. It will be necessary to print these forms in order to sign them.
Digital Signatures WILL NOT be accepted.

Ballot Rejection Notices must be uploaded along with ID to our secure site here.
Note: updates to voter registration information must be done IN PERSON if updating during the late registration period (30 days before) of an election.
REGISTRATION FORMS
Voter Registration Application - Use to register to vote in Gallatin County, name changes and change of address.
Request for Cancellation of Voter Registration - Use if you have moved out of state or wish to cancel your registration with Gallatin County.
BY-MAIL/ABSENTEE BALLOT FORMS
Request to be Added/Removed from Absentee List - Use to request mail ballot (optional - revert to polling place voter). NOTE: *This form CANNOT be used to update residential address*
BALLOT-RELATED FORMS
Ballot Rejection Notice - complete this form and upload along with acceptable ID. Use if your absentee ballot has been rejected due to signature verification reasons (mismatched signature or lack of signature)
Replacement Ballot Request - If your by-mail ballot has not been received.
NOTE: If within 10 days of election, it is recommended to come in person to request a replacement ballot due to mailing time.
